I see gentoo is currently using man-1.5.  

IMHO man-db, as used in eg debian and suse, is the better choice.

Has anyone worked on an ebuild for man-db?

I've started one, but onyl hve the most obvious parts done -- I still
have to read up on the details of writing ebuilds....
